Care Navigators are trans, non-binary or intersex (TNBI) community members who work alongside the Sussex Gender Service to offer you help and advice.

Who are Care Navigators?

Care Navigators are trans, non-binary or intersex (TNBI) community members who work alongside the Sussex Gender Service to offer you help and advice. The role is still under development and welcomes your feedback, but the aim is to support you as you journey through the service pathways and help you access information about the Sussex Gender Service. They can support you with a variety of issues while you are seen by the service, through group and individual support.

Care Navigators are employed by The Clare Project and work closely with the Sussex Gender Service, but are independent of the NHS.

Care Navigator Referral Form

Please fill in this form if you would like to arrange an appointment with a Care Navigator from the Clare Project and Sussex Gender Service. Care Navigators may be able to assist you short-term with non-clinical issues which may be affecting your journey through the service pathway, providing information about: help with deed polls, obtaining a Gender Recognition Certificate, connecting with community, housing and finance issues, communicating your needs with your GP, changing GP and more. Once you have submitted the form, a Care Navigator will be in touch within 5 working days.

Sussex Gender Service Drop-in

Every other Monday: 11:00am-1:00pm
The Ledward Centre, 14a Jubilee St, Brighton BN1 1GE

This drop-in run by the Care Navigators is an opportunity to meet staff from the Sussex Gender Service, ask questions and give feedback about Sussex Gender Service. It is open to anyone looking for information about the service and Care Navigators are available to speak to in group or privately.

Sussex Gender Service Online Drop-in

Every other Wednesday (alternating weeks with the in-person drop-in): 5:30pm-7:30pm

Care Navigators with lived experience host this fortnightly online drop-in to ensure you have a safe and accessible space to ask questions, give feedback, and learn more about the Sussex Gender Service and what you can expect. You can chat and bring questions or feedback in a group, or ask to speak to a Care Navigator privately in a breakout room.
