The Care Navigator is a vital member of the Clare Project and the Sussex Gender Service Pilot, providing support and guidance to individuals accessing the service. The service is person-centred, and Care Navigators offer practical support, non-clinical advice, and represent the needs of service users through effective communication with differing groups to support individuals and ensure that the needs and experiences of the service users are at the forefront. The Clare Project is dedicated to providing person-centred, non-clinical support to the Trans, Non-Binary and Intersex community in Sussex (TNBI).

The Sussex Gender Service (SGS) is an NHS adult gender service pilot that provides local transgender healthcare for trans and non-binary people. The SGS team assesses and diagnoses gender incongruence, offering referrals and signposting to other services. The SGS team is experienced and multi-disciplinary, with clinical support from the gender identity clinic at Nottingham Centre for Transgender Health. The SGS also has care navigators with lived experience who provide support and signposting to local organisations.

This is a hybrid role, involving work at our office in Brighton/Hove, remote work, service users’ homes, and various community venues. Please consider this travel requirement when applying for the position.
